Elective cosmetic procedures are not covered by insurance. The first consultation is complimentary. The charge for a second consult is $150.00. Please be aware that when scheduling your surgery, a 20% (and/or cost of implants) non-refundable deposit is due.
The balance of the surgery charges are due 14 business days prior to the surgery date. We provide a number of payment options, which may be used individually or combined according to your wishes. Cash, Check, Visa, Master Card, Discover, American Express, Financing Plans
CANCELLATION/ RESCHEDULING POLICY:
We understand that a situation may arise that could force you to postpone your surgery. Please understand that such changes affect not only your surgeon but other patients as well. Dr. Tavallali’s time, as well as that of the operating room staff, is a precious commodity, and we request your courtesy and concern.
You may reschedule your surgery once at no charge, with at least 14 days notice. Your deposit will apply to your rescheduled surgery date if it falls within 1 year of the original surgery date. Should you need to reschedule again, there will be a $1000 fee to do so.
We cannot reschedule surgery within 13 days of the date of surgery. Should you need to reschedule 13 days before surgery you will lose your deposit and/or cost of implants and any applicable credit card or financing company service fees.
If you cancel your surgery within 14 days, your 20% deposit and/or cost of implants and any applicable credit card or financing company service fees are not refundable. We will refund any payments except the 20% deposit and/or cost of implants and any applicable credit card or financing company service fees.
If you cancel your surgery within 7 days of the date of surgery you will lose 50% of the surgical fee paid as well as any applicable credit card or financing service fees.
Some surgeries are performed in the hospital and surgery center. Please be aware that the hospital and anesthesia departments are independent from each other and Dr. Tavallali. You will be responsible for making appropriate arrangements with these businesses.
Hospital and anesthesia fees must be paid in full 7 business days prior to surgery, otherwise the hospital reserves the right to cancel your surgery.
If revisionary procedures, hospital visits and/or stays, or if you seek medical treatment by other physicians, there will be additional fees, including the cost of the operating room, supplies and anesthesia, which would be your responsibility.
