     The plastic surgery techniques and procedures for buttock enlargement or enhancement have made substantial strides over the past few years. Previously only silicone elastomers were available for enlargement of the buttocks. These silicone blocks led to firm buttocks with a high rate of complications and I no longer use them due to patient dissatisfaction.

 

     Over the past 5 years a new technique using the patient’s own fat has gained popularity especially amongst Brazilian plastic surgeons and has come to be known as the Brazilian buttock augmentation or lift.

 

     The procedure involves liposuction of several liters of fat from the patient’s hips, thighs and abdomen and injection of the fat into the buttocks using multiple passes of a small tube to place the fat in linear channels under the skin and in the buttocks muscle. The surgery is done as an outpatient surgery  under general anesthesia and, and takes about 2-3 hours to perform. Recovery is speedy but a garment must be worn for several weeks and direct pressure on the buttocks must be avoided.

 

     Although it is expected that some of the fat cells will die and be absorbed, 70-80% are expected to survive and be incorporated into the body as normal fat cells; growing larger with weight gain and getting smaller with weight loss. However, since a sculpting has also been performed of the torso, this result will always stay and not be affected by weight fluctuations.

How long does the surgery take?

Depending on the amount of fat to be injected into the buttocks, the surgery can take from 2-3 hours.

 

How long is the recovery?

Most resume normal activities within 4-5 days. No sitting is allowed for a few days and no exercise is allowed for two weeks.

 

What are the risks of the surgery?

The major risks of the surgery is infection which occurs uncommonly. Bleeding is also a  small risk. Some of the fat is expected to resorb over time which may lead to a diminished result in some patients.

 

What type of anesthesia is used?

The surgery is usually performed with general anesthesia, though smaller injection can be done under local tumescent anesthesia.

 

What technique is used?

Tumescent liposuction is performed with a technique that allows maximum viability of the fat cells. The cells are then separated from the other fluids and blood with which they are aspirated, and then injected into the buttocks in a linear manner within the fatty tissues as well as the skin layers.

 

Is there much discomfort with the surgery?

Patients are sore for a few days after surgery but the pain is controlled easily with medications.
